The Northern Health Care Management (NHCM) MI Choice Waiver program is a division of Northern Lakes Community Mental Health which provides home and community based services for people who are elderly and/or disabled in 10 counties:
- Antrim
- Benzie
- Charlevoix
- Emmet
- Grand Traverse
- Kalkaska
- Leelanau
- Manistee
- Missaukee
- Wexford
Through this program, eligible adults who meet income and asset criteria may receive Medicaid covered services, like those provided by nursing homes, in their own homes or another residential setting such as adult foster care homes or homes for the aged. MI Choice Waiver program staff can provide assessment, care management, and purchase services in the community using Medicaid dollars.
Available Waiver Services
Participants can receive the basic services that Michigan Medicaid covers and one or more of the following available services, which are unique to the Waiver Program:
- Adult Day Care
- Chore Services
- Counseling Services
- Home Delivered Meals
- Homemaker
- In-Home Respite Care
- Out-of-Home Respite Care
- Personal Emergency Response System
- Private Duty Nursing
- Training
- Transportation (non-medical)
- Environmental Accessibility Adaptations
- Specialized Medical Equipment & Supplies
Through person-centered planning, the clients and case manager collaboratively determine which services are appropriate.
- Medicaid/Eligible for Medicaid
- Eligible for nursing home admission
- Over 18 and disabled or elderly
- Live in 10 counties listed above.
How to refer someone to the MI Choice Waiver program?
